訪問エリアは当診療所のある港区から渋谷区、新宿区、品川区、目黒区、世田谷区、千代田区などの診療所を中心とした半径16km圏内が対象となります。ご自宅が対象エリアかどうか分からない場合は、お答えしますのでご連絡ください。
24時間365日いつでも安心をお届け出来る体制です。専門的な医療機関から在宅に移った時に呼吸や痛みの管理が自宅で出来るのかどうか不安になっている方も多いと思います。そのような時に医師、看護師を中心としたチームが24時間体制でサポートしていきますのでご安心ください。
The visitation area extends from our clinic in Minato Ward to areas including Shibuya, Shinjuku, Shinagawa, Meguro, Setagaya, Chiyoda, and other clinics within a radius of approximately 16 kilometers. If you are unsure whether your home is within the designated area, please contact us, and we will provide the necessary information.
We operate a system that ensures peace of mind 24 hours a day, 365 days a year. Many individuals may feel concerned about managing breathing and pain at home when transitioning from specialized medical facilities to home care. In such situations, our team, consisting of doctors and nurses, is available around the clock to provide support, offering reassurance to those in need.
訪問診療は健康保険が適用されますので、ご負担される医療費には上限があります。
原則、ご高齢者は、診療費の1割（医療保険は一部3割、介護保険は一部2割）をご負担いただきます。高齢受給者証および、後期高齢者証をお持ちの方は、自己負担金額が定められています。
例えば、ご自宅にお住まいで、70歳以上の方が月に2回訪問診療をつけた場合の自己負担額の目安は以下のとおりです。

Home visit medical care is covered by health insurance, so there is a limit to the medical expenses you are responsible for. In general, elderly individuals bear 10% of the medical expenses (health insurance covers a portion at 30%, and long-term care insurance covers a portion at 20%). For those with a senior recipient card or a later-stage senior recipient card, the amount of self-payment is specified.
For example, for individuals aged 70 or older living at home, the estimated self-payment amount for receiving home visit medical care twice a month is as follows (the amount may vary for the first month):
